Early identification of special educational needs is an important part of ensuring that the best provision is made for all children and young people. Where a child or young person continues to make less than expected progress, despite high quality differentiated and targeted teaching, it is important to understand whether there is an underlying need which is acting as a barrier to learning.

 

Within Barnet we have developed our SENCO Toolkit which is a resource to support SENCOs with the early identification and assessment of special educational needs. The toolkit consists of the following sections:

The Barnet 'My Support Plan' is a template for children and young people who have been identified at SEN Support. It is a best practice model which reflects the four stage cycle of Assess-Plan-Do-Review.

The plan ensures that the child or young person is at the centre of the support plan, and involves parents/carers and the child and young person at the earliest stage with a person-centred planning component in the form of a one page profile.

Some children and young people with more complex needs may need support above and beyond what is ordinarily available in a setting; this might mean that the pupil would need an Education, Health and Care Needs Assessment (EHCNA).

 

Before a request for an EHC Needs Assessment is made, the child or young person would normally have in place a Support Plan that shows how everyone has worked together, including outside agencies, to identify and support the pupil's needs, and also identifies how the pupil has been supported using resources which are normally available in the setting.

Here you will find all the guidance and documents you need to prepare for and undertake an Annual Review meeting. Following this section, there are also useful resources and videos to support your understanding of the annual review process and statutory timeframes. 

Before the Annual Review meeting

It is important that the following documents are completed, gathered and circulated to all those involved in the Annual Review meeting. This should be done at least two weeks before the Annual Review meeting takes place:

  • Views of the child/young person
  • Views of the parents/carers
  • Professional Advice and Reports (completed by all professionals involved with supporting the child or young person)
  • Summary Advice (completed by the school or setting)

All these documents will contribute to the Annual Review of the EHC Plan; therefore, it is essential that those involved in the meeting have the opportunity to read through them beforehand.

 

Following the Annual Review meeting

The Report on the Annual Review of an EHC Plan should be completed after the Annual Review meeting has taken place. This form, along with the other documents and any supplementary reports, must be sent to the Local Authority within the statutory 2 weeks of the Annual Review meeting date.

Please note: if the EHC Plan has been issued by or is under the responsibility of another Local Authority (e.g.: Enfield or Haringey), you will need to ensure that you complete the annual review documentation relevant to that Local Authority. If you are unsure or have any questions about this, contact your Barnet SEN Caseworker.
