For young people with an EHC Plan their plans for preparation for adulthood short term and long-term outcomes are reviewed. There is a review of support in school for those with additional needs but no EHC Plan.

 

For young people going to university evidence for DSA funding can be considered and there is a need to make sure the needed assessments are in place.

 

Health & Social Care continue with transition planning.  A Transition Plan will be carried out if a young person is likely to have needs deemed eligible under the Care Act 2014.  The Social Institute Institute for Excellent (SCIE) have developed a useful set of resources to help local authority staff, social workers, young people, parents and carers to plan for the transition to adult care services.  

 

The Preparing for Adulthood Transition Planning and Support protocol details what should happen for young people in Years 12 to 14, when it should happen and who is responsible.

 

The Department for Education (DfE) fund the Preparing for Adulthood programme (PfA) and their mission is that young people with SEN and Disabilities have equal life chances as they transition into adulthood.
